When filling out a paper application please be sure to fill out the entire form, including filling in all bubbles and initialing where required. Underneath the signature on an adult application there is a bubble that states "YPT completion certificate attached and Background Check Authorization form attached."

The council will not accept any application, youth of adult, that is not complete. All information must be filled out and all applications must be signed. In addition, YPT certificates and CBC authorization forms must be attached to all adult applications. Any application that is not complete will be sent back with the individual who brings it to either the Findlay or Lima office.
